Care Consultation Information

Care Consultation is a collection of services designed to assist persons with Alzheimer’s disease or related dementias, and/or their families, in planning for and dealing with all aspects of the illness experience. Delivery of services can be via telephone, email, or in-person contact.

Care Consultation is not case management, care management, counseling or case advocacy, though part of the Care Consultation is to identify the need for, and refer families to, the appropriate agencies or persons that provide these services and meet these needs, including local Association professionals.

Key Features of the Care Consultation include:

  • Assessment of needs
  • Assistance with planning and problem solving
  • Support services, including:
    • Active Listening
    • Recommendations
    • Information and Referral
    • Education of Disease Process and Interventions
    • Follow Up consultations, as indicated.
