Health and wellness fairs offer a great opportunity for your company, organization, house of worship, and/or community event to provide information about Alzheimer's disease and the programs and services available through the Alzheimer's Association!

To request the Alzheimer's Association's participation at your next health fair, please complete the PDF Health Fair Request Form which can be submitted via email or mailed in to:

Once your form has been received, a staff member will contact you to confirm the Association's participation.

While we strive to accommodate any request, a thirty-day advance request is appreciated. For bilingual audiences, a forty-five day advance notice is appreciated.
